Care Plan Philosophy

At Emmaus House the emphasis is placed on high quality individual person centred care with attention to individual service users needs, preserving their dignity and identity at all times.

To ensure that we deliver the highest possible service provision each service user is treated as a unique individual with a plan of care that reflects their specific needs.

On admission an accurate and comprehensive assessment will be carried out and will be followed up with regular reassessments as often as the service user's status demands. Care plans will be reviewed at least once a month to reflect any changes in personal needs and will comply with the Latest Standard of Quality & Safety.

Residents will be actively encouraged to be involved in the planning of their care at all times and if they wish they may also involve members of their family, friends, advocates and other agencies in the planning and delivery of care.

In our endeavour to provide people with optimal quality of life, comfort and dignity, care plans will be unique and take account of any special conditions such as e.g. Alzheimer's disease, Dementia, Nutritional problems to name but a few.

Finally, it is important to remember that the purpose of the care plan is to guide all who are involved in care to provide the best possible service therefore care plans should contain all the information needed to care for each individual.
